Honoring Choices Florida Program Convener
Utilizing Respecting Choices®, an internationally recognized, evidence-based advance care planning (ACP) model, Community Hospice & Palliative Care serves as the program coordinator to support sites with implementation plans and spread this model in Northeast and North Central Florida—the first of its kind in the state. Community Hospice & Palliative Care engaged the support of local hospitals as initial community partners.
Community Hospice & Palliative Care guide and support efforts in health and community organizations who want to develop ACP programs and facilitate ACP with those they serve. Six Northeast Florida hospital systems participate and community and organizational partnerships continually develop to implement the program outside of the hospital settings. We want to inspire people in our community to have conversations regarding health care choices in the event of a sudden or chronic illness.
